I hereby give permission for my child to take part in all camp activities, Including sports and games (Unless otherwise indicated below) and absolve Abundant Life Baptist Church and Cornerstone Baptist Temple and or/it's camp staff from liability to me or my child because of injury received while attending or traveling to/from  Pleasant Valley Ranch. In case of any accident or serious illness, I hereby authorize the camp to call upon a physician of their choice and follow his or her instructions. I, the camper, will abide by the established rules and regulations of the camp, I understand that my misconduct could result in my dismissal with no refund.
